Foundation Check Request

The Barton School Foundation is a 501(c)(3) nonprofit corporation. The purpose of the Foundation is to provide financial support for the mission of Clara Barton Open School.  If you have expenses that should be paid through the Foundation, you may submit Foundation Check Request form (see below).

When purchasing things that are going to be submitted to the Foundation for reimbursement, please use the Foundation's tax ID number (20-4885206) and do not pay sales tax on reimbursable expenses.  It may not seem like much on one purchase, but it adds up and we could save the Foundation hundreds of donation dollars by doing this.

To get reimbursed or request checks for payments from the Barton School Foundation:

  1. Download the Check Request Form and complete it on your computer.

  2. Save a copy on your computer for your records.

  3. Print the completed form.

  4. Attach all receipts or invoices documenting the expense (place the documentation in an envelope if there are more than a few items).  Note that receipts should only include items being submitted for reimbursement -- please separate your items when you check out at the store, if possible.

  5. Place the form and receipts in the Principal’s mailbox in the Main Office.

Please note that check requests have to be approved by the Principal, and then one of the parent volunteer Foundation board members must come in to the school to write the check.  Typically this takes about a week, but it could take longer depending on when people are in the building, so plan accordingly.  Also note that requests at the end of the school year should be in at least a week before the last day of school.
