PARENTS - IMPORTANT: PLEASE READ!

We have a number of students who are not yet up-to-date on their immunizations,which are required by Minnesota state law.
If you have received a phone call or a letter indicating that your child’s immunization record is incomplete, and you have not yet contacted the health office, please note that your child may be excluded if the shot record is not received at school, as soon as possible. Exclusion can be avoided by sending us the clinic document showing the dates the shots were given OR by telling us what the appointment date is that you have set up for your child to get the shots. DO NOT DELAY!
DEADLINE: MONDAY, NOVEMBER 7!
AFTER THIS DATE, DO NOT SEND YOUR CHILD TO SCHOOL IF YOU HAVE NOT
GIVEN US THE CLINIC SHOT RECORD or
GIVEN US THE APPOINTMENT DATE or
SUBMITTED A CONSCIENTIOUS EXEMPTION FORM.
These are UNEXCUSED absences.
NOTE: Only families of students who are not up to date have received letters/phone calls. If you have not received a letter or phone call, do not panic. It means your child already has all the shots he/she needs.
If you wish to conscientiously object to any immunization, click here. Scroll to the 2nd page and look for Conscientious Exemption on the right. This form needs to be printed out, notarized by a notary public and returned to school. Important: Be sure to specify which immunizations you are objecting to.
